Teachers,
This is a form I've used with my students for Science Fair projects. I would have students fill in the dates I gave them, and the parents would sign and return it. Throughout several weeks, students would turn in one component at a time. I would help them revise each component and return it to them. It was the student's responsibility to edit each component before it was placed on the final backboard. I did a couple of class experiments and walked them through the entire process, so they were familiar with each component. So that parents could HELP (not do everything) at home, I sent a list of the component definitions. Everything below except the proposal would be compiled for the final project.

*Note: The proposal will tell you whether or not the student is on the right track. If their project sounds like a demonstration rather then an experiment, he/she must change their project. Do not allow them to do further work until they've altered their topic. Many demonstrations can easily be turned into experiments. Just be sure that the student is testing variables.

Students are assessed on each of these components, as well as a final backboard grade and a presentation grade. For the backboard grade, students must have all necessary components. The data has to be typed and the appearance should be organized and neat. Pictures are a must for those entering the Fair.

The presentation grade is based on the student's ability to demonstrate an understanding of material being presented as well as familiarity with testing and data collected. The student must be able to clearly outline the procedure used, explain the results of the data collection, and draw logical conclusions based on the results to include whether their hypothesis proved true.
